Headstone/Marker Setting Benefits: 
Pursuant to Indiana Code 10-17-10-12, a $100 benefit is available only for the setting of a federal headstone for the veteran. 

The surviving spouse or next-of-kin must sign the "Statement for Setting of Federal Headstone" form acknowledging they were informed of this benefit, and that if applying for this benefit, payment shall go to the said business setting the marker. 

A copy of the headstone/marker application (VA Form 41-1330) shall be attached as well as a copy of Honorable Discharge, DD 214, or Report of Separation.
